What we believe

Better care is achieved when data, clinical expertise, and human support work together

Conditions like kidney disease and heart failure affect more than a chart. They shape daily decisions, relationships, and access to care. Healthmap works with partners to make support more timely, practical, and connected across the health journey.

Our Purpose

Chronic disease requires a dynamic approach

Chronic disease shapes daily decisions, relationships, access to care, and the lives people hope to lead. Healthmap was founded by kidney-disease stakeholders, patients, physicians, patient advocates, and healthcare executives who believed that more compassionate, life-improving kidney care is both possible and necessary.

Today, our mission-driven team draws on experience across health plans, payer services, providers, and population health to help people with kidney and cardiac disease live healthier, longer lives while supporting the value-based results our partners expect.

Built on data. Centered on people.

A provider- and patient-driven approach to population health management

Healthmap’s roots are in technology and data. Our Kidney and Cardiac Health Management programs use proprietary technology, clinical expertise, and complex care management to identify high-risk disease earlier and support clinically appropriate next steps.

We work with providers and patients to plan care transitions proactively and optimize renal replacement therapy, including in-home dialysis and transplant when clinically appropriate. The result is a scalable approach that can integrate with existing infrastructure and workflow processes.

Trusted expertise

Built to support health plans, providers, and the people they serve

Founded in 2013, Healthmap is a trusted partner to health plans and to primary care physicians, nephrologists, cardiologists, endocrinologists, pharmacists, dialysis providers, and other specialists across the country.

Our Compass platform and corporate network domain and workstations are HITRUST CSF® certified. Healthmap has also received NCQA Population Health Program 3-year accreditation for Kidney Health Management, and Prevalidation status for Population Health Management. We are a proud affiliate member of America’s Health Insurance Plans and are endorsed by the National Kidney Foundation of Florida.
