Healthmap Solutions Supports Kidney Care National Plan, Calls for Turning Priorities into Everyday Care

Healthmap Solutions (Healthmap) has expressed support for the Kidney Care National Plan in a response outlininghow its priorities can translate into everyday care for people living withchronic kidney disease (CKD).

Tampa, FL, October 29, 2026 – Healthmap Solutions (Healthmap) has expressed support for the Kidney Care National Plan in a response outlining how its priorities can translate into everyday care for people living with chronic kidney disease (CKD). Healthmap emphasized that earlier identification, access to effective treatments, and expanded kidney transplantation must be paired with education, care coordination, and support to help patients overcome barriers to care.

The response, “From Policy to Practice: Healthmap Solutions’ Perspective on the Kidney Care National Plan,” is authored by Jonathan Gavras, MD, FCCP, Chief Medical Officer, and Stephanie Toth-Manikowski, MD, MHS, FASN, Chief Nephrology Officer. It describes how a comprehensive approach to kidney health management can help put the Kidney Care National Plan’s priorities into practice, with a focus on preserving kidney function and supporting informed treatment decisions.

Healthmap’s response highlights four priorities for translating the plan into practice:

1. Identify CKD and intervene earlier. Identify patients while there is still kidney function to protect and connect them with recommended treatments that can delay or slow disease progression.

2. Connect innovation with treatment access. Address affordability and other barriers so eligible patients can initiate and adhere to effective therapies.

3. Begin transplant conversations sooner. Identify patients appropriate for evaluation before they require dialysis and provide and implement education and support for patients, care partners, and potential living donors.

4. Build a comprehensive foundation for kidney health. Pair guideline-directed care with nutrition, physical activity, weight management, smoking cessation, and management of conditions that can accelerate CKD progression.

Through its Kidney Population Health Management program, Healthmap engages members with CKD Stages 3, 4, 5, Unspecified, and end-stage renal disease (ESRD), identifies recommended treatments likely to slow disease progression, and supports members with education, care coordination, and advocacy. The company also helps identify members appropriate for transplant evaluation and supports them throughout the evaluation process.
