Improving Kidney Health Management: A Conversation with Eric Reimer
On the Becker’s Healthcare Podcast, Healthmap CEO Eric Reimer discusses why health plans should invest in kidney health management and what makes a program successful.

In an episode of the Becker’s Healthcare Podcast from Becker’s Healthcare, Healthmap Solutions CEO Eric Reimer discusses why health plans should invest in kidney health management and what it takes to build a program that works. The episode runs about 15 minutes.
Why health plans should invest in kidney health
Chronic kidney disease (CKD) is common, often undiagnosed until late stages, and closely tied to diabetes, hypertension, and heart disease. For health plans, members with advanced CKD and end-stage renal disease account for a disproportionate share of medical spending, much of it from avoidable hospital stays. Eric makes the case for investing earlier, before members reach dialysis. (See The Value of a Kidney Population Health Management Program: The Health Plan’s Perspective.)
Elements of a successful program
Eric outlines what a successful kidney health management program needs. Across his interviews, the core elements are consistent: identifying the right members early, working with the physicians members already trust, addressing the whole person including comorbidities and social needs, and tying the program’s success to measurable clinical outcomes and savings. Eric covers each of these in more depth in Groundbreaking Population Health Strategies for Chronic Kidney Disease.
