MA PLANS DISRUPTING PROGRESSIVE KIDNEY DISEASE

2020 brings not only a new decade but a significant challenge for Medicare Advantage (MA) plans: The 21stCentury CURES Act takes effect January 1, 2021, and one of the most complex, high risk, and high-cost patient populations will now be able to enroll in MA plans.  At Healthmap Solutions, one word describes our approach to preparing for and successfully managing this population on behalf of MA plans: DISRUPTION.

 

ESRD: Costly and complex

This fall, over 600,000 previously ineligible End-Stage Renal Disease (ESRD) patients will have the opportunity to choose to enroll in an MA plan. ESRD is the most serious stage of Chronic Kidney Disease (CKD), a progressive disease with no cure. Providing care for ESRD patients can cost from $80,000 to $100,000 per patient each year. But this is just the tip of the iceberg.

 

Disrupting the ESRD pipeline

There are 37 million Americans living with CKD, the precursor to ESRD. Many of these people will never know they have CKD until they crash into dialysis. Millions of others are receiving treatment for “CKD gateway” diseases, such as diabetes, hypertension, heart disease, and obesity.

 

DISRUPTING the progression to ESRD  with effective quality of care management and initiatives is critical to helping patients maintain and improve their quality of life while also maintaining costs..
